Transportation Program

For curb to curb livery service rides through CTS:

• Person must be registered with ALS CT and live within CT.
• Person must not have any other alternative transportation services available (such as transportation available through Medicaid or the VA).
• Person must complete the below Transportation Application before transportation can be scheduled.

For van rentals:

• Contact your Care Services Manager to book transportation
• Van rentals are dependent on availability and funding

The transportation program accommodates both non-wheelchair and wheelchair users. The program may be used for round-trip rides to and from ALS Clinics and/or Neurology offices in CT and may also be used for other types of medical appointments and quality of life events such as weddings, holidays, children's events, etc. Rides are approved based on available funding.

Once you have completed the application and receive confirmation from your Care Services Manager that you are enrolled in the ALS CT Transportation Program, it is the responsibility of the person living with ALS or their caregiver to contact CTS to schedule transportation at least 7 days in advance.

Please call CTS directly at 888-450-4329 or e-mail ALSRides@ctstransit.com to make a transportation request. Please be prepared to provide CTS with the following information: Location, Date, Time & Duration of your Appointment. Should you have any questions about the arrangements of your ride or issues following a ride, please contact CTS directly.

Van rentals for trips are currently covered by ALS CT on a limited basis. Please contact your Care Services Manager for more information.

ALS CT is unable to help fund the purchase of an accessible vehicle.
