Transportation Program

We offer both curb to curb livery service and van rentals. People living with ALS can obtain transportation free of charge to clinic appointments, other medical appointments and quality of life rides through Coordinated Transportation Solutions (CTS). Van rentals through various providers are also available. This program is dependent on available funding.

Eligibility Criteria

For curb to curb livery service rides through CTS:

Person must be registered with ALS CT and live within CT.
Person must not have any other alternative transportation services available (such as transportation available through Medicaid or the VA).
Person must complete the below Transportation Application before transportation can be scheduled.

For van rentals:

Contact your Care Services Manager to book transportation
Van rentals are dependent on availability and funding

How to Make a Request through CTS

Once you have completed the application and receive confirmation from your Care Services Manager that you are enrolled in the ALS CT Transportation Program, it is the responsibility of the person living with ALS or their caregiver to contact CTS to schedule transportation at least 7 days in advance.

Please call CTS directly at 888-450-4329 or e-mail ALSRides@ctstransit.com to make a transportation request. Please be prepared to provide CTS with the following information: Location, Date, Time & Duration of your Appointment. Should you have any questions about the arrangements of your ride or issues following a ride, please contact CTS directly.

Van Rentals

Van rentals for trips are currently covered by ALS CT on a limited basis. Please contact your Care Services Manager for more information.

Accessible Van Purchase

Purchasing an accessible van can be very expensive but is often considered when anticipating the future. If your family is exploring this option, you are encouraged to reach out to a company to help you navigate the questions concerning this investment. ALS CT is unable to help fund the purchase of an accessible vehicle.
